1. INSTRUCTIONS (Read Carefully)

	To qualify to enter the Science Fair, all portions of this form must be completely filled out.
	A Confirmation Page will be automatically generated by this process.  You should retain this confirmation sheet.  You may need to furnish a copy of it (with signatures) for your teacher.  This page is not required for Cobo Hall.  A duplicate of this confirmation page is available whenever the entry is 'modified' which may occur at a different location.
	If there are no additional SRC forms required or if the project is in the Junior division, no hard copy should be submitted.  The electronic form will be used.
	Review the Phase One Judging Checklist and the rules to avoid disqualification.

2. SCIENTIFIC REVIEW COMMITTEE PRE-APPROVAL (JR. AND SR. DIVISIONS)

Be Aware, if you use human subjects, vertebrate animals, human or animal tissue, recombinant DNA, or pathogenic agents in your project, you must have obtained prior SRC approval.

3. DIVISION - CLASSIFICATION

On the Following Form, select  Grade, and Classification. After the entry form deadline, your decision is final. Absolutely no changes to classification will be allowed once the entry form deadline has elapsed.  Online changes to grade, classification, title, and all information is permitted until the entry form is 'finalized'.   Consult the Science Fair Newspaper for a complete list of project classification descriptions.
